• An identification card or driver’s license issued by the California Department of Motor Vehicles must be current or issued within the last five years

• A United States passport

• An inmate I.D. card issued by the California Department of Corrections and Rehabilitation, if the inmate is in custody in California state prison

• Any form of inmate identification issued by a sheriff’s department, if the inmate is in custody in a local detention facility

• A valid consular identification document issued by a consulate from the applicants country of citizenship, or a valid passport from the applicant’s country of citizenship

• A driver’s license issued by another state or by a Canadian or Mexican public agency authorized to issue driver’s licenses

• An identification card issued by another state

• A United States military identification card

• An employee identification card issued by an agency or of office of the State of California, or an agency or of office of a city, county, or city and county in California

• An identification card issued by a federally recognized tribal government

* In the event the signer does not have acceptable ID, 2 witnesses who are not named in the document and who do not have a financial interest in the document and who also have valid ID can be used as an alternative method to ID the signer. 

* You must have a completely filled out document. A blank and/or incomplete document will not be notarized.

* The name that is being signed on the document must match the legal name of the individual signing on his/her I.D. card. Names on documents that do not legally correspond with an individual’s I.D. card will not be notarized.
